This form must be completed by the Biological Parent or Legal Guardian who wishes to enrol their child/children into Centurion College.

The following documents must be submitted with this form for the application to be processed. Photocopies WILL NOT be made at school.

  • Birth Certificate or Identification Document
  • Progress Report and Transfer Letter from Previous School
  • Copy of Parents/Guardian Identification Document
  • 2x ID Photos of learner
  • Proof of Payment
